Home Hospital Teaching


The Vista Unified Home Hospital Teaching program is available to students who require instruction at home for at least 5 consecutive weeks for up to one semester.  To initiate Home Hospital Teaching, the parent will need to coordinate with the School Nurse.

Parents will be asked to provide:

  • The MEDICAL REPORT FOR HOME INSTRUCTION completed and signed by the physician.



  • The APPLICATION FOR HOME TEACHING/ENGLISH (EL FORMULARIO en Español) -completed by the parent/guardian.


  • For Mental Health/Behavioral Health diagnoses only, also have Physician/Therapist complete the TREATMENT PLAN FORM.


  • Middle and high school students will need to meet with their counselor to develop a class schedule that will be appropriate for Home Teaching.

Special Education students will need to have an IEP to determine Home Teaching placement.
